Welcome to the Chapel of the Cross Preschool family! We are a Christ-centered, project- and play-based preschool for children ages 3–5. Our mixed-age class promotes leadership, independence, and creativity, allowing children to grow at their own pace in a loving, faith-filled environment.
Below you will find all the forms and information needed to enroll your child for the upcoming school year. Please follow our simple three-step registration process to secure your child’s spot.

Download & Complete Required Forms
Please click, download, and digitally fill or print, then scan the following documents. All of these forms must be filled out completely to finalize your child’s registration:

Submit Your Completed Packet
You can use a digital form filler to complete the documents on your device, or print and scan them. Email or drop off the completed printed files to the preschool.
Once we receive your paperwork, we will be in touch with you to confirm your child’s spot. We truly can’t wait to welcome your family into our classroom!
